Annual Membership Form 2008
In order to maintain and provide an up-to-date directory and list of members to Central VSNA. We request you to renew your annual membership and update your information, preferably before March 15th.
Those of you that have already renewed your membership, and filled the membership form to update your information, thanks for your cooperation.
Please use the form below to renew or begin VSNA-TX membership.
Annual Family Membership Fee: $100.00
Central VSNA Life members pay $65.
Note: $35 will be sent to Central VSNA for membership, and $65 will be utilized for VSNA -TX Chapter activties.
